Mounjaro dose schedule & titration: why the first month is low-dose

A patient-friendly explanation of titration: starting at 2.5 mg weekly, when doses increase, and why slow-and-steady...

Titration is the planned, gradual increase in dose over time. For tirzepatide, it’s designed to improve tolerability, not to ‘hold you back’.

Typical titration (overview)

  • Start: 2.5 mg once weekly for 4 weeks
  • Then: 5 mg once weekly
  • If needed: increase in 2.5 mg steps after at least 4 weeks on the current dose
  • Maximum: 15 mg weekly

Why start low?

  • Most side effects happen at the start or during dose increases.
  • A slower ramp often means fewer drop-outs and better long-term results.
  • It gives you time to learn eating patterns that reduce nausea.

Don’t self-adjust your dose

Changing the dose or schedule without clinician advice can increase side effects or reduce effectiveness. If symptoms are tough, talk to a clinician about slower titration and symptom management.
