Mounjaro Dosing Guide: Complete Week-by-Week Schedule

Understand Mounjaro's titration schedule from the 2.5mg starting dose to the 15mg maximum, including injection tips and...

Mounjaro (tirzepatide) uses a gradual dose escalation approach designed to minimise side effects while allowing your body to adjust to the medication. This guide explains the standard dosing schedule and what to expect at each stage.

Understanding Mounjaro Dosing

Unlike some medications where you start at the full dose, Mounjaro requires a titration period where you gradually increase your dose over approximately 20 weeks. This approach helps:

  • Reduce the severity of gastrointestinal side effects
  • Allow your body to adjust to the medication
  • Help your prescriber find the optimal dose for your needs

Important Notes About Dosing

The 2.5mg Starting Dose

The initial 2.5mg dose is not intended to produce weight loss. It's designed purely to help your body adjust to the medication. Don't be discouraged if you don't see results in the first month.

You May Not Need the Maximum Dose

Not everyone needs to reach 15mg. Some patients achieve excellent results at 10mg or 12.5mg. Your prescriber will work with you to find the dose that provides good results with acceptable side effects.

Dose Adjustments

If you experience significant side effects at any dose, your prescriber may:

  • Extend the time at your current dose before increasing
  • Reduce your dose temporarily
  • Suggest strategies to manage side effects

How to Inject Mounjaro

Mounjaro comes in pre-filled KwikPen devices. To administer:

  1. Choose your injection site: Abdomen (at least 5cm from navel), front of thigh, or back of upper arm
  2. Rotate sites: Don't inject in the same spot twice in a row
  3. Clean the area: Use an alcohol swab and let it dry
  4. Inject: Insert the needle at a 90-degree angle, press the button, and hold for 10 seconds
  5. Dispose safely: Place used pens in a sharps container

Timing Your Weekly Injection

You should inject Mounjaro on the same day each week. You can take it with or without food, at any time of day. Many people find it helpful to:

  • Set a weekly reminder on their phone
  • Choose a day that's easy to remember (e.g., every Sunday)
  • Inject in the evening if nausea is a concern (so they sleep through the initial hours)

What If You Miss a Dose?

If you miss a dose:

  • If it's within 4 days of your scheduled dose: Take it as soon as you remember
  • If more than 4 days have passed: Skip the missed dose and take your next dose on your regular day

Never take two doses at once to make up for a missed dose.

Storage Requirements

  • Before first use: Store in the refrigerator (2-8°C)
  • After first use: Can be kept at room temperature (up to 30°C) for up to 21 days
  • Do not freeze: Discard if frozen
  • Protect from light: Keep in original packaging until use

Comparison with Wegovy Dosing

Mounjaro has more dose steps than Wegovy:

  • Mounjaro: 6 dose levels (2.5, 5, 7.5, 10, 12.5, 15mg) over 20+ weeks
  • Wegovy: 5 dose levels (0.25, 0.5, 1.0, 1.7, 2.4mg) over 16+ weeks

The more gradual titration with Mounjaro may help some patients adjust more easily to the medication.
